You may register with us as an NHS patient if you are resident in our catchment area as outlined below.
You will be required to complete some paperwork.
How to register:
- Please provide photo ID such as a driving licence or passport and a proof of address.
- If you have never been registered with a GP in England you will need a valid passport with 6 months valid visa (if applicable).
- If you are registering a child under the age of 5, please bring their red book or immunisation record.
- After 3-4 working days your details will have been entered on to our system and you will also be able to book appointments as normal.
We cannot prescribe medication for you until you have seen a doctor. PLEASE NOTE ANY MEDICATION THAT IS TAKEN AND NOT PRESCRIBED OR KNOWN TO THE WHALEBRIDGE PRACTICE IS TAKEN AT THE PATIENT RISK AND THE PRACTICE CANNOT BE HELD RESPONSIBLE FOR ANY HARM.
Please therefore use the below checker to ensure that you are within our current registration boundary.
Existing Patients :-
Due to a recent change in our contract. The changes concern patients who live outside of the practice boundary.
We are sorry to inform you that we would suggest that you register at a practice nearer to yourself. Please note that we are no longer contractually obliged to home visit.
